FORMER STUDENTS...WHERE ARE YOU?
Attention former students! Did you leave Basic High School/Academy between 2013-2016 (but didn’t graduate) and are you now attending school somewhere else? If so, please contact us in the registrar’s office! Let us know where you are attending now. We can help you! Make sure your new school has your transcripts! Contact Ms. Helland at 702-799-8000, ext 4066.
Students who have graduated from Basic Academy must now request transcripts at transcipts.ccsd.net They are no longer available at Basic Academy.
Official Transcripts: Typically required by most colleges/universities for admissions. May also be required for some job applications. Official transcripts from Basic Academy come in a sealed school envelope that you can address to the entity you need to send it to.
Unofficial Transcripts: Unofficial transcripts are typically used as personal copies, for some job applications and some scholarships. Unofficial transcripts are usually NOT accepted by colleges/universities when applying for admission. A transcript is considered unofficial if an existing official seal has been broken or the transcript is missing the Registrar's signature or school seal, if required.
STEP 1 – Complete the CCSD School ONLINE Registration Form
Parents are to visit the CCSD website and complete the ONLINE registration form. This online step replaces the previous CCF-705 form. There are two registration options (shown below): one is for students currently enrolled in a CCSD school, the other is for students new to CCSD.

STEP 2 – Visit the school's Registrar's Office
After completing the online registration form, parents should then visit the Registrar's Office. To make the completion of the registration process simpler, parents are advised that they should come prepared with the following documentation:
1. Immunization Requirements - ALL STUDENTS MUST BE COMPLIANT WITH DPT, POLIO & MMR VACCINATIONS
DPT/DT - 4 required - 4th dose must be on or after 4th birthday
Tdap - required except if less than two years since last DPT shot
Polio - 4 required - 4th dose must be on or after 4th birthday
MMR - 2 required - 1st dose on or after 1st birthday, 2nd dose at least 4 weeks apart
New students to Nevada are also required to have the following immunizations (returning CCSD students are encouraged to receive these immunizations):
Hepatitis A - 2 required, separated by 6 months or doctor verification of disease.
Hepatitis B - 3 required, total series covers 16 weeks or doctor verification of disease
Meningitis vaccine - 1 dose required
Varicella (chicken pox) - under 13 years of age, 1 required; over 13 years of age, 2 required separated by 2 months or doctor verification of disease

Must present any one of the following: a utility bill (water, gas, power or garbage) or a rental/purchase agreement. Document submitted MUST be current and in the parent's name. TELEPHONE/ CELL PHONE BILLS ARE NOT AN ACCEPTABLE FORM OF PROOF OF RESIDENCE.
4. Affidavit of Residence
Only required if student is living with someone other than parent. Students in this situation may not enroll in classes until this documentation is provided.
5. Guardianship Documentation
Only required (when applicable) if student is living with someone other than parent. Students in this situation may not enroll in classes until this documentation is provided.
6. Academic Records
If new to the CCSD, an unofficial transcript (preferred), most recent report card or withdrawal documentation from previous school is acceptable.
